Congressional Certificates

To request a Congressional Certificate, please complete and submit the request form below with the requisite information. Congressional Certificates will only be provided to individuals or organizations based in the 12th Congressional District and to individuals or organizations who have made substantial contributions to the 12th Congressional District.

Please note that the submission of a request does not guarantee a congressional certificate. Requests must be received at least two weeks prior to the date by which they are needed.

What are these options?
